First Appointment

Your appointment is the time reserved specifically for you. Coming prepared for your initial appointment gives you more time with our team and to ask as many questions as you need. Please take this opportunity to complete the paperwork below.

Please also bring the following:

• Photo I.D.
• Dental or Medical History Records, including medication you may be taking
• Insurance Carrier Card

 

We advise you to arrive 15 minutes prior to your scheduled appointment time.

Insurance

We believe that everyone deserves quality dental care and a beautiful smile, and we will do everything we can to help our patients! We accept most insurance plans and will bill all our services to ensure you maximize your benefits.  We kindly request you familiarize yourself with your individual insurance plan. Your specific policy is an agreement between you (or your employer) and your insurance company. Our accounting staff will prepare and submit all the necessary forms for your dental benefits.

As a friendly reminder, you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. We do not render our services on the basis that insurance companies will pay all of our fees. We are here to serve you and help improve your oral health, so we base our diagnosis and treatment on what is best for you!
