Our mission at Northwest Periodontics is to always treat our patients with respect while providing a safe, warm and welcoming environment where you receive the highest quality periodontal care. On this page you will find important information about our office and what to expect during your initial visit.

Appointments

Dr. Halpern is currently accepting new patients. Please call us at 770-928-6655 to schedule an appointment. Once you make an appointment at our office, that time will be reserved for you. If, for some reason, you are unable to keep your scheduled appointment time, call us with 48 hours notice to reschedule.

What to Bring to your First Appointment

To provide you with prompt treatment please be sure to bring the following when you visit our practice for your initial visit:

  • Photo Identification
  • Any dental or medical records pertaining to your treatment (including medications that you are taking)
  • Insurance Information/Card

Please arrive 15 minutes before your scheduled appointment time to fill out necessary paperwork. If you would like to complete your paperwork prior to your appointment, you may print and fill out the appropriate forms found below.

Below you will find our new patient forms. Please print and complete the forms before your first visit with us. While we do not require that you complete this paperwork prior to visiting our practice, this will save you and our staff time when you come in for your appointment.

Coming well-prepared for your initial appointment will assure that we have all of the information needed to provide the best possible care for you. Please educate yourself on your particular signs and symptoms by taking a look at the great information on this site. Insurance

Forms in Woodstock, GA

We accept most dental insurance plans. Please familiarize yourself with your individual insurance plan. Our accounting staff will prepare and submit all the necessary forms for your dental benefits. However, we remind you that your specific policy is an agreement between you and your insurance company. We will accept insurance assignment if we have a written pre-determination from your insurance company and you will be required to pay the difference at the time of service. Should you decide to have treatment performed before a pre-determination has been completed by your insurance, you will be asked to pay 50% of the balance of the time of treatment. Accounts unpaid after 60 days from the date of service will have late payment charges added to the bill.

You are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. We do not render our services on the basis that insurance companies will pay all of our fees. We base our treatment on what is best for you.

Payment

Payment is expected at the time services are rendered, unless other arrangements have been made. This includes any co-payments or deductibles (amount not covered by your insurance).
